In order to receive support services for students with disabilities you are required to provide current medical or assessment documentation from an approved professional to Disability Services, preferably three months in advance of your anticipated program start date.
Documentation should be dated and signed on official letterhead by a professional qualified to make the diagnosis (e.g. Registered Psychologist) with the diagnosed disability stated clearly and thoroughly (e.g. DSM-IV code). It should also be current (i.e. within 3-5 years for ADHD/Learning Disability, 1 year for Mental Health Disability). Where applicable it should include a list and description of all diagnostic tests used in the evaluation with the relevant test scores used to document the disability and describe relevant educational and medical history. The documentation should describe how the disability and treatment, if applicable, impacts the individual’s functioning in an academic setting and include specific recommendations for accommodations or services. Please note that neither IPPs (Individualized Program Plans) nor documentation on prescription pads are sufficient at the college level. In addition, Disability Services may request further assessment and/or documentation.
